NELSON S. ROMAN, United States District Judge: Plaintiff Spencer T. Crumbsie, who is currently held in the Westchester County Jail (“WCJ”), brings this action pro se, asserting claims of federal constitutional violations. He filed his complaint while he was incarcerated in the Elmira Correctional Facility. Plaintiff seeks damages and sues: (1) WCJ Correction Officer Warthwood; and (2) two unidentified “John Doe” WC] Correction Officers. The Court construes Plaintiffs complaint as asserting claims under 42 U.S.C. § 1983, as well as claims under state law.
By order dated February 6, 2025, the court granted Plaintiff’s request to proceed in forma pauperis (“IFP”), that is, without prepayment of fees.' The Court directs service on Warthwood, directs Warthwood to comply with Local Civil Rule 33.2, and directs the Westchester County Attorney to provide to Plaintiff and to the Court the full identities, service addresses, and badge numbers of the unidentified “John Doe” defendants.
' Prisoners are not exempt from paying the full filing fee, even when they have been granted permission to proceed IFP. See 28 U.S.C. § 1915(b)(1).
DISCUSSION A. Service on Warthwood Because Plaintiff has been granted permission to proceed IFP, he is entitled to rely on assistance from the Court and the United States Marshals Service (“USMS”) to effect service.2 Walker v. Schult, 717 F.3d. 119, 123 n.6 (2d Cir. 2013); see also 28 U.S.C. § 1915(d) (“The officers of the court shall issue and serve all process . . . in [IFP] cases.”); Fed. R. Civ. P. 4(c)(3) (the court must order the USMS to serve if the plaintiff is authorized to proceed IFP).
To allow Plaintiff to effect service on Warthwood through the USMS, the Clerk of Court is instructed to fill out a USMS Process Receipt and Return form (“USM-285 form”) for Warthwood. The Clerk of Court is further instructed to issue a summons for Warthwood and deliver to the USMS all the paperwork necessary for the USMS to effect service of a summons and the complaint upon Warthwood.
If a summons and the complaint is not served upon Warthwood within 90 days after the date that a summons for that defendant is issued, Plaintiff should request an extension of time for service. See Meilleur v. Strong, 682 F.3d 56, 63 (2d Cir. 2012) (holding that it is the plaintiff’s responsibility to request an extension of time for service).
Plaintiff must notify the Court in writing if his address changes, and the Court may dismiss this action if Plaintiff fails to do so.
2Although Rule 4(m) of the Federal Rules of Civil Procedure generally requires that a summons be served within 90 days of the date the complaint is filed, Plaintiff is proceeding IFP and could not have effected service until the Court reviewed the complaint and ordered that any summonses be issued. The Court therefore extends the time to serve Warthwood until 90 days after the date that any summons for him issues.
B. Local Civil Rule 33.2 Local Civil Rule 33.2, which requires particular defendants in certain types of prisoner actions to respond to specific, court-ordered discovery requests, applies to this action. Those discovery requests are available on the court’s website under “Forms” and are titled “Plaintiff’s Local Civil Rule 33.2 Interrogatories and Requests for Production of Documents.” Within 120 days of the date of service of a summons and the complaint, Warthwood must serve responses to those standard discovery requests. In his responses, Warthwood must quote each request verbatim.3 C. Unidentified “John Doe” Defendants Under Valentin v. Dinkins, a pro se litigant is entitled to assistance from the Court in ascertaining an unidentified defendant’s identity and service address. 121 F.3d 72, 76 (2d Cir. 1997). In the complaint, Plaintiff supplies sufficient information to allow the Westchester County Department of Correction (“WDOC”) to provide the full identity, service address, and badge number of each of the unidentified “John Doe” defendants; those defendants include any correction officer assigned to the WCJ on or about August 28, 2024, who assisted Warthwood, on that date, when searching Plaintiff in the alleged manner described in the complaint. It is therefore ordered that the Westchester County Attorney, who is the attorney for and agent of the WDOC, must ascertain the identity, service address, and badge number of each of the abovementioned unidentified “John Doe” defendants. The Westchester County Attorney must provide this information to Plaintiff and to the Court within 60 days of the date of this order.

Within 30 days of receiving this information, Plaintiff must file an amended complaint naming each of the newly identified individuals as a defendant and providing his or her service address and badge number. The amended complaint will replace, not supplement, the original complaint. An amended complaint form that Plaintiff should complete is attached to this order.
Once Plaintiff has filed an amended complaint, the Court will screen it and, if necessary, issue an order directing service on the newly identified defendants and, if appropriate, directing their compliance with Local Civil Rule 33.2.
CONCLUSION The Court directs the Clerk of Court to mail an information package to Plaintiff.
The Court also directs service on Defendant Warthwood.
The Court further directs the Clerk of Court to: (1) issue a summons for Warthwood; (2) complete a USM-285 form with the service address for Warthwood; and (3) deliver all documents necessary to effect service of a summons and the complaint on Warthwood to the USMS.
The Court additionally directs Warthwood to comply with Local Civil Rule 33.2 within days of the date of service of a summons and the complaint.
The Court directs the Clerk of Court to mail a copy of this order and a copy of the complaint to the Westchester County Attorney, at the Westchester County Law Department, Michaelian Office Building, Suite 600, 148 Martine Avenue, White Plains, New York 10601.
An amended complaint form is attached to this order.
